I don't see this form on atx...client received one with boxes 8,9,10, and 11 filed out....any help would be appreciated on how to file this on the 1040....

Can anyone clarify whether annual distributions from a qualified plan which are taxable distributions be included in earned income when figuring the additional child tax credit.
