The client letters are included with the forms.

It is on page 29 of the 1040 instructions on IRS website.

You need to put the amount on line 10c of "Line 1(Sch A(1040))-Medical and Dental Expenses"

I registered online on 9/28 and had no problem and was assigned my original number. I have since received a postcard in the mail verifying registration.

And may I add #3.........if you want to sound legitimate and professional......check your grammar and spelling....geez, dude, did you graduate high school???
