(I've got a customer who faxes me the authorization). Also what are the forms that need to be signed? Are there 2 authorizations to be signed plus the 8879? kcjenkins Posted March 3, 2009 Report Share Posted March 3, 2009 I've used their Drivers License for the ID. Perhaps your client could fax you a copy of that? There is a FeeCollect authorization that has to be signed, the same form that asks for the ID on page 2. Also there is a Consent to Disclose and a Consent to Use. Those forms you fill in a pin number, and print them, but they do not have to be signed. That's it, other than the 8879, and state form if required.
